Mock Survey


Our Mock Surveys help prepare your facility for the State Survey performed by your state’s regulatory office.  During this confidential audit, SAGE will not only identify problems within your facility, but we will also recommend solutions for correction.   Once the audit is complete, SAGE will have an Exit Conference to verbally review all findings with the key personnel and will provide a written report outlining those findings and recommendations for correction.

Our audits take a minimum of 5 days and we will visit the facility at all times of the day to make sure each shift is reviewed.  Our audits can be announced or unannounced, whichever you please.

Compliance Consulting for Assisted Living Facilities

SAGE offers consulting on both operations and compliance, with the main focus on revising or improving your facilities current practices. Consulting services, which include observations and recommendations, can be targeted toward any of the elements within a Mock Survey, a specific department or for an entire facility.  A common request from past clients is to have SAGE study and recommend improvements for Time Management.

After observing current practices, SAGE will have an Exit Conference with key personnel to review findings and create a plan for training and implementing SAGE's recommendations for a more efficient or compliant system/process. SAGE will also follow up with monthly or quarterly maintenance visits to ensure continued, appropriate implementation of SAGE's recommendations is occurring.

Drafting a Plan of Correction


Deficiencies cited by a surveying agency require a Plan of Correction (POC) they approve and accept.  SAGE's reputation and experience with developing effective and accepted correction plans is unparalleled.  We create unique facility-specific plans that, if correctly implemented, not only correct the deficient practice but will improve overall facility practices and operations.

A facility must write an individual Plan of Correction for each deficiency received during an actual state survey. It must include how the plan will be monitored and by whom.  Implementation of any deficiency correction plan is critical since repeat deficiencies are even more grievous and frequently result in licensure enforcement action.  Consistency, monitoring and on-going correction of the deficiency is absolutely necessary.  Systematic and routine audits by our highly skilled team of experts can be written into your POC.  SAGE's proven track record will ensure your Plan of Correction will be appropriately implemented and consistently adhered to.

We suggest a similar Plan of Correction be developed to address issues identified during one of our mock surveys.  SAGE can help draft a Plan of Correction after an actual state survey and/or after a mock survey.

Post-Survey Implementation of the Plan of Correction (PoC)


The Post Survey Implementation consists of setting up internal systems and processes that specifically target the deficient practices found during the Mock Survey. It includes systems for ongoing monitoring of the implementation of the Plan of Correction as well as revising what does not work.

SAGE can perform this after an actual state survey and/or after a mock survey.

Policies and Procedures


SAGE can provide your facility, whether brand new or looking to update, Operational Policy & Procedures which uniquely, follow the content layout, by chapter, of the state rulebook.  These policies and procedures have been vetted time and time again during actual surveys!

Our policies and procedures can be customized to include your logo, mission statement and whatever else you feel needs to be included to brand your community.

Each department, such as Dietary, Maintenance, HIPPA and Pharmacy are outlined and addressed.
If ordered, you will receive SAGE's Operational Policy & Procedures, with Applicable Forms, in both a hard copy version as well as an electronic copy. 

Our policy manuals are available in searchable PDF format so when an issue does arrive, you can find it quickly and effortlessly.
